If you have already registered through LifeLegacy, we will need to update your information and send you new registration cards. Please email us your full legal name, current mailing address and date of birth to info@sciencecare.com. Please indicate in the email that you had previously registered with LifeLegacy.

If you or your loved one were already guaranteed placement into LifeLegacy’s program, please submit the guaranteed acceptance forms to info@sciencecare.com and that guarantee will still be honored.
